Burial Requirements at Clark Veterans Cemetery

INCOMING BURIALS AT CLARK VETERANS BURIAL FROM OUTSIDE THE PHILIPPINES

The required method for return to the Philippines for Burial at CVC is as follows:

   1. The Transfer of Ashes must be hand-carried with all Required Documents including the Consulate Clearance that should be presented upon Arrival or Air Freighted to the Philippines for Burial at Clark Veterans Cemetery The Funeral Home should assist you in attaining the Following Documents for Inurnment or Interment Burial

   2. You should have received One (1) or Two (2) Registered Original Death Certificates

   3. Fifteen (15) True Attested Copies of the Death Certificate should have been requested.

Note: Please do not make copies of the Original Death Certificates. Most times they are rejected as being Fraudulent.

   4. Cremation Permit issued by City Hall

   5. Medical Examiner or Coroner Certificate Stating that the Death was of Natural Causes

   6. Cremation Certificate issued by the Crematory Stating that Cremation was held on this Date

   7. Transfer Permit from Place of Death to Clark Veterans Cemetery at Clark Freeport Zone in Angeles City Pampanga Philippines (Not Manila or Clark)

Note: The Transfer Permit must be as stated as above for the Cemetery or Burial will be Denied.

Submission to Philippine Embassy/Consulate in country of death for entrance to the Philippines

Notify the Consulate before submitting any documents to verify if there are any additional requirements due to the current conditions.

* Submit the Following:

    1. Three (3) true Attested Copies of the Death Certificate

    2. Copy of the Cremation Permit

    3. Copy of the Medical Examiner or Coroner Certificate

    4. Copy of Cremation Certificate

    5. Copy of Transfer Permit from Place of Death to Clark Veterans Cemetery at Clark Freeport Zone in Angeles City, Pampanga Philippines.

    6. Photo ID of the Deceased Passport or Two (2) IDs

    7. Deceased Birth Certificate

    8. Flight Information or Air Fright Man: Date of Departure to the Philippines Arrival Date at either Manila or Clark

    9. Passport ID of Person carrying the ashes back to the Philippines Note: If the Deceased Wife is carrying the Ashes the Consulate may ask for the Marriage License

The Funeral Home will prepare and submit for approval to the Consulate the Required documents and the Air Freight Manifest to the Philippines for an Interment Burial.

Upon issuance by the Consulate which could take Two (2) to Three (3) Weeks for the Clearance and Approval to Entry the Philippines.

Please check and verify that the Consulate Mortuary Certificate States that upon arrival at either Manila or Clark and upon receiving Immigration and Customs Clearance Stamps for The Ashes or the Casket of the Deceased will be transferred to Clark Veterans Cemetery at Clark Freeport Zone in Angeles City Pampanga Philippines for an Inurnment or Interment Burial.

DEPARTURE TO THE PHILIPPINES

   1. Ashes must be in a Plastic Transfer Urn or a Cremation Shipping Box An urn can be purchase here

   2. The Transfer Urn must be placed inside of a Box or in a Cremation Box that must be wrapped in white paper and sealed with clear tape with copies of the Documents inside and attached to the outside of the Box

Note: Inside the Box:

    1. One True Attested Copy of Death Certificate

    2. Copy of the Cremation Permit

    3. Medical Examiner or Coroner Certificate

    4. Cremation Certificate

    5. Transfer Permit

Outside the Box (Tape on the sides)

     1. Copy of the Cremation Certificate

     2. Transfer Permit

Tape to the Bottom in a White Envelope:

    1. True Attested copy of the Death Certificate

    2. Copy of the Medical Examiner or Coroner Certificate

    3. Cremation Permit

Upon Arrival at Manila or Clark a Quarantine Certificate Clearance MUST BE STAMP on an Original Death Certificate.

Note: If you do show the Documents and do not receive a Stamp upon Arrival or the Transfer Permit does not name the Cemetery, the Burial will be Denied.

Those missing Documents will cause a one to two week delay in the burial until they are obtained in Manila which can be very expensive.

For the Incoming Inurnment Burial at Clark Veterans Cemetery and for my Office the following Documents are Required for submission

   1. Two (2) DD 214 please verify that Honorable is on the Copy being submitted

   2. Two True Attested copies of Death Certificate

   3. Two (2) Copies of the Original Death Certificate with the Quarantine Certificate Stamp

   4. Two (2) Copies of the Copy of the Cremation Permit

   5. Two (2) Copies of the Medical Examiner or Coroner Certificate

   6. Two (2) Copies of the Cremation Certificate

   7. Two (2) Copies of the Transfer Permit from Place of Death to Clark Veterans Cemetery at Clark Freeport Zone in Angeles City Pampanga Philippines

   8. Two (2) Copies of the Consulate Clearance

For an Incoming Interment Burial at Clark Veterans Cemetery and for my Office the following Documents are required for submission

  1. Two (2) DD 214 please verify that Honorable is on the Copies being submitted

  2. Two True Attest Copies of Death Certificate

  3. Two (2) Copies of the Original Death Certificate with the Quarantine Certificate Stamp

  4. Two (2) Copies of the Medical Examiner or Coroner Certificate

  5. Two (2) Copies of the Transfer Permit from Place of Death to Clark Veterans Cemetery at Clark Freeport Zone in Angeles City Pampanga Philippines 

  6. Two (2) Copies of the Consulate Clearance

Death Occurred in the Philippines

For Interment Burial (Casket) at CVC

  1. A Certified True Copy of the Original Death Certificate

  2. DD 214 with Honorable Discharge

  3. Disinterment Burial (Casket) Permit obtained from Angeles City Registrar for the Burial of the Veteran at CVC

  4. If required, a transfer permit from the Veterans’ place of Death to Clark Veterans Cemetery, at CFZ, to Angeles City, Pampanga

For Disinterment with Interment Burial (Casket) at CVC

  1. Certified True Copy of the Original Death Certificate

  2. DD 214 with Honorable Discharge Disinterment (Casket) Permit obtained from Place of Disinterment

  3. Interment Permit from Angeles City Registrar for the Burial of the Veteran at CVC

  4. If required, a transfer permit from the Veterans’ place of Disinterment to Clark Veterans Cemetery, at CFZ, to Angeles City, Pampanga

Note: If the Deceased Veteran is in a vault the entire vault can be transferred to the receiving cemetery (Clark Veterans Cemetery). If there is no vault, open, or in poor shape, the Deceased Veteran must be cremated under Philippine Sanitation Codes.

For Inurnment Burial (Urn)

  1. A Certified True Copy of the Original Death Certificate

  2. Cremation Permit

  3. Cremation Certificate

  4. DD 214 with Honorable Discharge

  5. A Disinterment Permit for the urn (If the urn has been buried) obtained from the Local City Registrar where the Veteran is buried

  6. A Re-interment (Urn Burial) Permit obtained from Angeles City Registrar for the burial of the Veteran at CVC

  7. If required, a transfer permit From the Cemetery where the Veteran is buried to Clark Veterans Cemetery, CFZ, Angeles City, Pampanga.

Note: Currently, Burials are held on Tuesday through Thursday with Military Honors

  There is no charge for the Burial or the Ordering and Placement of Headstone

  There are no Burials on Saturday or Sunday

  There are no Military Honors on Monday and Friday

  Only Emergency or Direct Burials on Monday or Friday

Non Veterans and Foreign Nationals

NOTE: Also, if any Veterans Organization outside of Angeles is need of a Flag, my office will provide one. We will LBC it to them if they send us the address where they want it sent.

The Only requirement is that they provide an Scan Copy of the Certified True Copy of the Death Certificate, the DD 214 with Honorable and a Copy of the Passport ID.
